I free-handed the design, and just appliquéd it onto a cheap t-shirt I got at Target. If you don't know how to appliqué, you can see my tutorial here.

LOVE your handiwork, Tara! She's definitely a cutie...and the shirt is darling. My current project is a scarf I'm knitting in PURPLE with a cable (first time). It's beautiful - and soft (50% merino, 50% silk. Time to get back to it...

My daughter would love one of those shirts. If you put one in your etsy shop in a size 8, I would definitely buy it (too subtle?)
